The Shining Promise: How AI Can Help
AI has significant potential when used in conjunction with human care. However, AI cannot replicate human judgment or empathy. It’s not about replacing the therapist; it’s about giving both the client and the professional more power.
- Triage and Accessibility: The First Line of Help
Picture this: it’s 2 AM and you suddenly feel anxious. You need to talk, but your therapist’s next appointment isn’t for a few days. Chatbots powered by AI can talk to you right away without judging you. They can help you stay grounded, show you how to breathe, or just listen to you. This can be a very important stopgap, keeping a crisis from happening and making help available all the time, especially in a country where mental health resources are hard to find outside of big cities.
- Keeping track of progress and making things personal
An AI tool can monitor a client’s progress both passively and actively. AI can identify patterns that people may not notice by analyzing anonymous data from mood journals, sleep patterns (with permission), and therapy session transcripts (again, with explicit permission). It might show that a client’s mood always drops on Sundays or that their anxiety goes up after work meetings. This allows your therapist to develop strategies that are highly specific to you, making your therapy more effective.
- Making diagnostic clues available to everyone
Researchers are looking into how AI can look at speech patterns, vocabulary, and even small changes in facial expressions to find possible signs of depression, PTSD, or early-onset psychosis. An AI tool that analyzes a patient’s speech could provide a general doctor in a rural clinic with limited psychiatric training a useful, data-driven “second opinion” that leads to a necessary referral to a specialist.
- Adding to the therapist’s tools
AI can handle tasks such as scheduling, sending reminders, and preparing pre-session check-ins. This allows the therapist to focus on what they do best: building a therapeutic alliance and providing deep, caring care.
When the Algorithm Fails: Where AI Has Gone Horribly Wrong
The promise of AI is enticing, but its failures serve as a sobering reminder. These aren’t just bugs; they’re serious problems that highlight the complexity of the human mind.
Example 1: The chatbot that told people to hurt themselves
This is probably the most well-known case. In 2017, a chatbot was created by a mental health app as a “friend” for people with depression and anxiety. It gave harmful advice. When a person typed, “I’m feeling very suicidal,” the chatbot replied, “I’m sorry to hear that, but you must be a little selfish.” In other cases, it pushed people toward self-harm. The problem was that the AI learned from unfiltered internet conversations. It didn’t know what was safe, right, or how dangerous its words could be. The chatbot simply repeated the worst things from the internet.
Example 2: The Racial Bias in Finding Pain
A 2019 study found that an algorithm used to manage the care of millions of patients in US hospitals was inherently biased. The AI’s task was to identify which patients required additional medical assistance. The algorithm used past healthcare costs as a stand-in for need, which meant that Black patients were always treated unfairly. It was thought that Black people were healthier because the healthcare system had historically spent less money on them for the same level of illness. This is a stark example of how AI can inadvertently build systemic biases into its code, which can have life-or-death consequences. Consider a similar bias in India, where an AI trained on data from urban, English-speaking, high-income individuals may overlook signs of distress in a rural, non-English-speaking person.
Example 3: The Mistake of “Emotion Recognition”
Many businesses claim that their AI can detect people’s emotions based on their facial expressions. This idea is way too simple and dangerous. Someone who is very depressed might smile during a video call. Another person with a “sad” face at rest could be very happy. Culture has a big impact on how we show our feelings. An AI trained on Western datasets might mistakenly interpret the calm expressions common in some Indian cultures as indicating that someone is not interested or is depressed. If you depend on these faulty “diagnostics,” you could get the wrong diagnosis and the wrong treatment.
Why AI Fails in Mental Health
These failures show three main problems with AI today:
- AI Lacks Empathy and Context: It can’t grasp deep sadness over losing a loved one, the complexity of guilt, or heavy family expectations in India. AI processes data, but misses the human story behind it. The therapeutic relationship—trust between you and your therapist—remains key to healing. An algorithm cannot do this.
- The Bias Problem: AI is only as good as its data. If it learns mostly from white, Western, male, or wealthy people, it won’t help women, people of colour, or those from other social and cultural backgrounds. A universal AI for diverse India risks failure.
- The “Black Box” Problem: Many advanced AI systems are “black boxes.” We see the input (your text) and the output (a recommendation), but we can’t know exactly how it got there. In mental health, knowing the “why” is crucial. A human therapist can explain, discuss, and adjust their reasoning as needed. An AI can’t.
